6 signs you may need a root canal treatment
Most people arrive at a root canal appointment having ignored the warning signs for weeks. That's understandable, the pain often comes and goes, and it's easy to assume a tooth that stopped hurting has healed. Unfortunately, an infected pulp doesn't resolve on its own, and a tooth that could have been saved simply with treatment can end up needing extraction.
Here are the signs worth acting on.
1. Pain that lingers after the trigger is gone
Healthy teeth can be briefly sensitive to something cold. The distinction that matters is duration. If you sip cold water and the sharp feeling disappears within a second or two, that's normal sensitivity. If it keeps throbbing for thirty seconds or longer after you've swallowed, the nerve inside the tooth is likely inflamed.
2. Sensitivity to heat specifically
Pain triggered by hot food or drink is a more specific warning sign than cold sensitivity, and it often indicates the pulp is already infected rather than merely irritated. Some patients find the pain eases if they hold cold water in the mouth, a distinctive sign that's worth mentioning to your dentist.
3. Pain when you bite down
If one particular tooth hurts under pressure, chewing, or just tapping it, the inflammation may have spread beyond the root tip into the surrounding bone. This often feels different from general soreness: patients can usually point to the exact tooth.
4. A tooth that has changed colour
A single tooth turning grey or brown, while the teeth around it stay the same shade, usually means the pulp inside has died. This can happen years after an injury to that tooth, and it's often painless, which is exactly why it gets missed.
5. A swollen or tender gum near one tooth
Swelling at the gum line above a specific tooth, sometimes with a small pimple- like bump that drains and refills, indicates infection finding a path out. The bump may come and go. It does not mean the infection has cleared.
6. Pain that wakes you at night
Pain bad enough to interrupt sleep, or that responds poorly to ordinary painkillers, warrants a same-week appointment rather than a wait-and-see approach.
What actually happens during treatment
The procedure has a worse reputation than it deserves. Under local anaesthetic, the infected pulp is removed, the canal is cleaned and shaped, and the space is sealed. For most patients it feels comparable to having a deep filling placed, the discomfort people associate with root canals is usually the infection they arrived with, which the treatment is what relieves.
Afterwards the tooth is more brittle than it was, so a crown is often recommended to protect it from fracturing.
When to come in
Book an appointment if pain has persisted more than two or three days, if you notice swelling, or if a tooth has changed colour. Catching an infection early is usually the difference between saving the tooth and losing it.
